HOW TO APPLY FOR SERVICE
Clients may contact FBC directly or can be referred by GPs, Community Health nurses etc. Clients are also referred from the Commonwealth Respite and Carelink Centre.
FBC is not always available to assist clients immediately due to funding constraints and there may be a waiting period before an assessment can be undertaken. Please be assured we make every effort to assist you as soon as possible.
At the time of assessment you will be asked a range of questions by the Assessor who will complete an assessment form, which is used to determine eligibility for services and the level & type of service required.
After the assessment you will be notified of the care commencement date & time. If for any reason the care cannot go ahead you will be advised of the reasons.
For holders of Department of Veterans Affairs White or Gold cards, application can be made for Veterans Home Care services to the assessment agency on 1300 550 450. For more information go to www.dva.gov.au/health/homecare/mainvhc.htm
WHO IS ELIGIBLE TO RECEIVE SUPPORT
You may be eligible to receive services from FBC if:
1. You are a person with a disability (physical, intellectual, mental or emotional impairment which is ongoing and functional, affecting day to day activities),
2. you are frail through age or illness,
3. you have dementia,
4. you have a family member with a disability,
5. you have a family member who is frail through age or illness,
6. you have a family member who has dementia,
7. your unpaid primary carer needs assistance in caring for you,
8. you are at risk of having to enter a residential facility such as a nursing home, but would prefer to stay living in your own home and would be able to do so if you had some assistance,
9. the use of regular support hours will help prevent a crisis situation that may mean you are able to avoid moving into a residential facility, either on a temporary or permanent basis,
10. you are the holder of a White or Gold card issued by the Department of Veterans Affairs.
PRIORITY OF NEED
FBC receives far more requests for support than we can meet and therefore we use a priority scale to determine which clients will be provided with support. Information about this priority scale can be obtained from your FBC Assessor.
If for any reason you choose not to accept service from FBC, you are able to reapply at a later date and your previous refusal will not influence the outcome of the later request.
Application to FBC does not preclude you from seeking services from another agency.
